NYC DOT Physical Examination

Paperwork required for pre-existing medical conditions

In order to ensure the DOT Physical goes as smoothly as possible, the following physical requirements should be adhered to:

  • If you are able to hear a loud whisper from across 5 feet at least without the need of a hearing aid, then you are eligible for the card. If not, you will be asked to sign up for an official hearing test and on passing, you will receive your card.
  • Both eyes have to be about 20/40 each while reading the Snellen Chart irrespective of whether you wear contacts or glasses or not. Drivers will also be tested on their ability to determine and distinguish between traffic lights.
  • If your blood pressure is between 140/90 and 180/100 you may be eligible for the card albeit for a short interval but only by your medical examiner’s leave. You qualify automatically if you have 140/90 blood pressure.
  • If you suffer from high blood pressure, please bring a list detailing your medications and the name of the doctor who prescribed
  • If you are diabetic, bring the medication list and name of the doctor who prescribed them. If you are a trucker please also bring the results from past blood sugar tests and average blood sugar levels as per the last 3 months (aka a hemoglobin A1C).
  • Bring a list of all prescription and over-the-counter medication you might be on such as allergy medication including dosage and strength.
  • If you have ever suffered a heart attack or have a history of cardiovascular disease, please also bring a completed stress test from your treating cardiologist.
  • If you suffer from sleeping disorders please bring 1 copy of test results that prove it if possible.
  • In case of blanks in the aforementioned information, keep the contact information of your doctors with you so that they can be asked to fill them.
